Village School Athletic Field

Welcome to TVS Athletics! To participate in TVS Athletics, we require that parents complete the following:

1 | EL2 & EL3 Forms Grades 6 to 12

To ensure our athletes are in peak condition, The Village School requires annual physicals in accordance with Florida High School Athletic Association (FHSAA) guidelines.

The FHSAA mandates that student-athletes complete a pre-participation physical evaluation (Form EL2) and be certified as physically fit to compete in interscholastic athletics. This physical remains valid for 365 days from the doctor signature date, after which a new evaluation is required for continued participation. You may download the EL2 Form here and take it to a participating physician.

Additionally, parents and students must submit a completed EL3 Consent and Release from Liability Certificate as part of the eligibility process. (FHSAA Bylaw 9.7)  The EL3 is an electronic form that you will complete by following the instructions below by creating an account at AthleticClearance.com. Your completed EL2 form must also be uploaded to this site.

2 | NFHS Required Videos Grades 6-12

All student-athletes in grades 6-12 are required to watch the following NFHS learn courses annually.  Please have your child watch the videos. Print the course completion certificates and upload the the account that you created at AthleticClearance.com.

Parents, please read the following important information:
  • I understand that if my child will not be attending or withdraws from this sport, I must notify the Athletic Department as soon as possible.
  • Athletics will bill my account for the amount due.
  • Every student-athlete (3rd to 12th grade) must have a sports release form completed for EVERY sport throughout the school year.
  • Student-athletes (6th-12th grade) must have updated FHSAA EL2 and EL3 forms on file through Athletic Clearance. This must be updated annually.
